If you are licensed in CA and want to originate in that state, you may be asked for a statement to be uploaded to your NMLS account. You will find the template for this statement attached here.
INSTRUCTIONS:
- Download and open the attachment
- Make sure the date at the top is updated correctly
- Fill out your full legal name as it appears in NMLS in the first grey box. Make sure to remove the default instructional text so that only your name is showing.
- Enter your NMLS # in the second grey box. Make sure to remove the default instructional text so that only your name is showing.
- Print and sign the document on the signature line provided.
- Upload where asked by the CA NMLS Team for review.
